Things you should know
In order for us to provide quality care, it is important that you are aware of and understand our policies & procedures and fee structure. Please see below for general information that will help guide you through the process. We can also provide you with detailed information on our policies, rates and fee structure with the intake packet once you decide to initiate services with us.
Insurance and Fee information
We are in-network providers with Premera, Regence, Other Blue-Shield Blue-Cross programs, Aetna First Choice Health and Cigna.
All other insurance carriers can be billed as out-of-network.
Please contact your insurance provider to verify and get authorization for your benefits before scheduling an evaluation.
We offer a private pay option for all our services.
Attendance and Cancellations
We ask that you provide in advance, a minimum 24-hour cancellation notice if you are unable to keep your therapy appointment for the day.
We expect at least 80% attendance in therapy over a 3-month period.

Privacy Policy
Federal regulation requires that healthcare providers inform their clients of the various circumstances when their medical and therapy related information may be used, disclosed, and how they can access this information. You will receive a copy of our policy with the intake packet.
